What qualifies me to be a provider on Enthea's network?

Only after execution of all 3 parts of the contract, i.e. Provider Agreement (part 1), Reimbursement Rates, Listing of Practitioners and Practice site(s), completion of credentialing, and after receiving committee approval, will clinics/providers be able to start receiving Enthea-covered patients and be considered an in-network provider with Enthea.
